Barrel Shortening
I’m not a gun smith, but I’m pretty sure there’s a right way and a wrong way to shorten a barrel. (Not my photo, just found it)

That was shot with another firearm.

Strange. Some metal folded in, some folded out. Looks like it happened right where the choke tube seats (see remnant threads are almost smoothed).
I suspect shooting with a loose choke, perhaps not seated. Petals of wad caught back edge of choke and tore it away.

That has been shot with two or more high velocity copper jacketed rifles rounds.
